Providing structured, clinically supported residential care for Veterans who require stabilization but do not meet criteria for inpatient psychiatric hospitalization.

Who We Serve

Hofunity Behavioral Health provides services for Veterans experiencing mental health conditions, including depression, anxiety, and PTSD, as well as those struggling with substance use disorders and co-occurring mental health and substance use conditions. Our program also supports Veterans facing housing instability or homelessness, and those who are having difficulty stabilizing in outpatient treatment alone, offering a structured residential environment designed to support recovery, stability, and successful reintegration into the community.

A BRIDGE BETWEEN INPATIENT AND OUTPATIENT CARE

Our program is designed to fill a critical gap in the continuum of care.

Hofunity Behavioral Health serves as:

  1. A step-down level of care for Veterans discharging from inpatient hospitalization or residential treatment programs (RRTP)

  2. A step-up level of care for Veterans who require more structure than outpatient services alone

We provide a structured residential environment focused on stabilization, recovery, and preparation for independent living.

Our community-based residential program provides:

  • 24/7 structured supervision and support

  • Medication monitoring and coordination with providers

  • Behavioral health stabilization and monitoring

  • Crisis prevention, intervention, and suicide risk monitoring

  • Life skills development and independent living support

  • Psychoeducation and relapse prevention

  • Transportation coordination for medical and behavioral health services

  • Discharge planning and transition to community-based care

Hofunity Behavioral Health helps address critical system gaps by providing:

  • Behavioral health residential placement for Veterans

  • Step-down stabilization following inpatient or RRTP discharge
