Note: Medical and Personnel records requests are processed within 72 business hours, excluding weekends and recognized holidays, after receipt of a complete and valid authorization. 
 

Requests for medical records require a completed Release of Information (ROI) prior to the release of records. To ensure the fastest response, the completed ROI must be submitted with the records request. Incomplete or improperly submitted requests may result in processing delays.

All records are released in accordance with HIPAA and applicable state and federal privacy laws.
