Hofunity Behavioral Health (HBH) LLC is a licensed Level II Behavioral Health Residential Facility (BHRF) providing short-term and extended residential treatment services to adults with behavioral health and substance use disorders, in accordance with Arizona Administrative Code standards.

HBH operates as a 24-hour, 7-day-a-week residential program within a structured, supportive, and recovery-oriented environment. Services are delivered by an interdisciplinary team of licensed and credentialed behavioral health professionals.

Target Populations and Age Categories

HBH serves adults ages 18 and older who are medically stable and appropriate for a residential level of care prior to transitioning to outpatient services. The program supports individuals with:

  • Serious mental health conditions

  • Substance use disorders

  • Co-occurring mental health and substance use disorders

  • Cognitive disabilities with comorbid behavioral health conditions

  • Older adults requiring integrated behavioral health and medical coordination

Residents must be able to participate in treatment planning, self-administer medications, and engage in structured programming. Specialized accommodations are assessed and implemented as clinically indicated.

Models of Care and Therapeutic Approaches

HBH utilizes an individualized, person-centered, and evidence-based treatment model that integrates behavioral health treatment, substance use recovery services, and life-skills development. Core models and approaches include:

  • Integrated co-occurring disorders treatment

  • Evidence-based curricula, including Living in Balance and The Matrix Model

  • Psychoeducation and relapse prevention

  • Skills training and development (ADLs and ILS)

  • 12-Step facilitation and AA/NA (on- and off-site)

  • Cognitive-behavioral and motivational approaches

  • Trauma-informed, culturally responsive care

  • Assistance with medication self-administration and psychiatric oversight

Treatment is delivered through individual counseling, group therapy, skills training, and coordinated medical and psychiatric services, with the involvement of external licensed providers when appropriate.

Licensed Providers and Practitioner Roles

Clinical services are provided by licensed and credentialed professionals, including:

  • Psychiatric Mental Health Nurse Practitioner (PMHNP)

  • Licensed Professional Counselors (LPC)

  • Licensed Independent Substance Abuse Counselors (LISAC)

  • Registered Nurses (RN)

Medical oversight and coordination are provided as clinically indicated.

Facility Staff

Facility staff provide 24/7 direct care, supervision, and support to ensure the health, safety, and well-being of residents. Facility staff include:

  • Behavioral Health Technicians (BHTs)

  • Behavioral Health Paraprofessionals (BHPPs)

Staff Experience

Our interdisciplinary team possesses experience in residential behavioral health and substance use treatment. Staff orientation and ongoing training include trauma-informed care principles, safety protocols, and culturally responsive practices.

Our 4-Phase Residential Program

HBH operates under a structured 4-Phase Residential Treatment Model, grounded in recovery-oriented care, dignity, cultural responsiveness, and empowerment. Program phases include:

Emotional stabilization and symptom management

Development of daily living and coping skills

Relapse prevention and application of learned skills​

Planning next steps and ongoing support after discharge.

Frequency and Intensity of Treatment

Programming is provided daily, including weekends and holidays. Expected treatment frequency includes:

  • Group therapy and psycho educational services: Daily

  • Skills training and development: Minimum of five (5) times per week

  • Individual counseling: 1–2 times per week or as clinically indicated

  • Treatment planning and clinical review: At least monthly or more frequently as needed

  • Psychiatric, nursing, and medical coordination: As ordered and clinically necessary

  • 24/7 supervision and support: Continuous
     

The minimum length of stay is 30 days. Continued stay beyond 30 days, including extensions up to or beyond 90 days, is based on ongoing clinical need, medical necessity, and documented monthly treatment reviews.

Residents are expected to participate in structured programming between 7:00 AM to 9:00 PM, with 24-hour staff supervision.

Residential Activities and Support

While HBH does not offer separate short-term respite services, residents have continuous access to supportive therapeutic routines throughout their stay. The residential environment emphasizes recovery engagement through structured groups, life-skills training, and individualized support.

Care Coordination and Discharge Planning

HBH emphasizes coordinated, recovery-oriented care through collaboration with Tribal and Regional Behavioral Health Authorities (TRBHA/RBHA), primary care providers, probation/parole entities when applicable, and community-based outpatient providers. Discharge planning and aftercare coordination begin at admission and are designed to support sustained recovery, housing stability, and successful community integration.
