Who We Serve
Hofunity Behavioral Health (HBH) provides 24-hour residential behavioral health treatment for adults (18 years and older) who have mental health and/or substance use disorders and are appropriate for a Behavioral Health Residential Facility (BHRF) level of care
Our program is designed for individuals who are medically stable, able to participate in residential treatment, and who can benefit from structured, person-centered services in a supportive environment.
OUR ADMISSION PROCESS

Referral & Initial Inquiry
Referrals to HBH are made by a licensed health care provider practicing within the scope of their license. Individuals, family members, or referral partners may contact our intake team to request information or begin the screening process
During the initial inquiry, our intake staff will:​

Review basic eligibility criteria

Discuss program services and expectations

Identify payer or payment options
-
Answer general questions about the program

​Admission & Orientation
Once eligibility is confirmed and documentation is complete, admission is scheduled based on bed availability. Admissions are generally processed on a first-come, first-served basis, with consideration given to clinical need and service availability
At admission, residents:

Receive orientation to the program and facility

Review program expectations, including the 30-day blackout period

Participate in initial assessments

Begin development of an individualized treatment plan.
Urgent or critical needs identified during admission are addressed immediately to ensure resident safety

​Screening & Eligibility Review
All prospective residents complete a person-centered screening to determine whether HBH can safely and effectively meet their needs. Screening considers behavioral health needs, substance use history, medical stability, and ability to participate in residential programming
Eligibility includes:

Age 18 or older
-
Behavioral health diagnosis appropriate for residential care
-
Medical stability and ability to self-administer medications
-
Ability and willingness to participate in treatment
-
Payer eligibility or self-pay approval
-
Agreement to program rules and minimum length of stay (30 days)
Certain exclusions may apply, such as the need for a higher level of care, acute medical instability, or safety risks beyond the scope of residential services

​Waiting List (If Applicable)
If no openings are available at the time of referral, individuals may be placed on a waiting list. When a bed becomes available, the intake team will contact the individual or referral source to proceed with admission

Medical Clearance & Required Documentation
Prior to admission, individuals must complete required medical clearances, including:

Medical clearance for residential admission

Tuberculosis (TB) screening within required timeframes

Physical examination within required timeframes
Completion of intake paperwork, consent forms, and required releases is necessary before admission can be finalized

Treatment Participation & Continued Stay
Residents participate in HBH’s structured, four-phase residential program, which includes counseling, skills development, recovery supports, health and wellness education, and coordination of care
Continued stay is reviewed periodically based on:

Progress toward treatment goals

Participation in programming
-
Ongoing medical and behavioral health needs

Clinical recommendations
