Hofunity Behavioral Health (HBH) is committed to ensuring timely, accurate, and confidential access to medical and personnel records in compliance with HIPAA, state, and federal regulations.
Medical Records Requests

Requests for medical records require a completed and valid Release of Information (ROI) prior to the release of records.

Medical records requests are processed within 72 business hours, excluding weekends and recognized holidays, after receipt of a complete and valid ROI. Incomplete or improperly submitted requests may result in delays.
